We are seeking a Registered Nurse Case Manager to join our team. The ideal candidate will have experience working in the hospice field.
Duties:
- Develop and implement patient care plans
- Collaborate with physicians and other healthcare professionals to develop comprehensive care plans
- Monitor patient progress and adjust care plans as needed
- Educate patients and their families on treatment options and disease management
- Advocate for patients and ensure they receive appropriate care
- Coordinate discharge planning and post-discharge care
Experience:
- Active RN license in good standing
- Minimum of 2 years of nursing experience in hospice or home health
- Experience in case management or care coordination preferred
- Strong communication and interpersonal skills
