Role Description
This role can be either full-time or part-time hybrid role as a Care Manager at Serenity Healthcare in Harlem, NY. As a Care Manager, you will be responsible for coordinating and overseeing the care of homebound patients. This includes assessing patient needs, developing care plans, coordinating with healthcare professionals, and ensuring the delivery of high-quality, patient-centered care. Additionally, you will provide education and support to patients and their families to promote better health outcomes.
