What are the responsibilities and job description for the Director of phase II position at Serenity Hills Life Center?
Position Summary:
The Phase II Director provides coordination and facilitation of programming for inpatient clients stepping down from the Phase I of treatment. The Phase II Coordinator collaborates in a multi-disciplinary setting to provide comprehensive treatment to the client as needed. Programming for Phase II will include Life Skills, Jobs Skills Training, Education Mentoring and Advocacy Linkage. The Phase II Coordinator helps support the clients in the second Phase of programming in their personal, education and professional growth.
Qualifications:
- Current Masters or bachelor's in education
- 3 year experience
- 3 year addiction experience preferred
Duties and Responsibilities:
- Develop, implement, and evaluate resident's progress
- Conduct classes sessions
- Collaborate with multidisciplinary team, community professionals, and community agencies to ensure continuity of care
- Direct and supervise the Mental Health Technician.
- Maintain at all times the confidentiality of former and current clients' protected health information, as directed by state and federal law, and program policy. Additionally, maintain confidentiality in any position with access to employee personal
- Attend all meetings per program requirement
- Complete all assigned mandatory trainings
- Perform other duties as assigned
- Provide trainings for mental Health Technician
