What are the responsibilities and job description for the Home Health LPN Field position at SERENITY HOME CARE?
The Home Health LPN provides skilled services to patients at home following the nursing plan of care established by the RN. Under supervision of a registered nurse, provides quality care to patients and performs technical skilled care in patient’s home in compliance with nursing standards and company clinical policies. This highly motivated LPN will ensure quality and safe delivery of home health services to patients/clients and provide accurate and timely documentation consistent with plan of care, assess and provide patient/client and family caregiver education and information pertinent to diagnosis and plan of care, maintain safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards, participates in coordination of home health services with Registered Nurse and clinical management. assists patient in learning appropriate self-care activities ,uses equipment and supplies effectively and efficiently. and documents and reports activity as required.
Coverage Area is mainly Ouachita, Union, Lincoln , Morehouse Parishes
Work Days : Monday- Friday with rotating weekend call
Must possess a current, unencumbered, active license to practice as an LPN in this state.
Current CPR Certification.
Minimum 1 year recent experience in a clinical setting. Home Health Experience Preferred
Must have a valid driver’s license and current automobile insurance.
