What are the responsibilities and job description for the RN Home Health Case Manager /Field position at SERENITY HOME CARE?
We are looking for a highly organized RN with strong nursing skills, excellent communication skills, and good planning skills . Also the ability to exercise initiative , independent judgment and willing to be a team player . Coverage area will include primarily Ouachita, Union, and Morehouse parishes.
As a Home Health RN you will administer skilled nursing care to patients requiring professional nursing service in their home.
- Making SN visits to assess/observe/intervene/educate , processing orders, notifying physician of patient needs and changes in condition
- Initiating care, completing certification/recertification orders
- Establish the Individualized Patient Plan of care
- Observing signs and symptoms and reporting to the physician and to other agency staff reactions to treatments, including drugs and changes in the patients physical or emotional condition or needs
- Referring Physical Therapist, Speech Therapist, Occupational Therapist, and Medical Social Worker, to patients requiring these specialized skills
- Following and implementing the agency's policies and procedures
- Preparing clinical records, care plans, progress notes and summaries of care conferences on patients in a timely manner as per agency policy Job Requirements.
Job Type: Full-time
Pay: $65,000.00 - $68,700.00 per year
