Serenity Home Health Care, LLC is a service provider that offer Case Managers that guide families through their long-term community care options across the state of South Carolina. This role is grounded in social work. The Case Manager is the liaison between services and the Participants.
Requirements
· Bachelor’s degree, experience in social work, 2 years of assessments history, care planning, communication skills, observations, evaluations, and be at least 18 years of old
· Pre-Employment Requirements: Clear sled background check, required documents, training, car insurance, TB test, driver’s license, and pass knowledge test, 10-year MVR
· Monthly contact with Participant (various types of visits including face-to-face)
· 30% average travel or more to Participants, meetings, and training using personal vehicle in a minimum of 1 entire area of 1 regional office.
· Time management, active listening, story teller, and problem solver
· Prioritize tasks, create and terminate services, work independently
· Willingness to learn, take constructive criticism, and make constant improvement
· Available from 8:30 a.m. to 5:00 p.m. Mon-Friday
· Case Managers cannot have other employment for more than four (4) hours between 8:30am and 5:00pm, Monday through Friday
