Overview
We are excited to announce the expansion of our early intervention services to include Case Management for children aged 3-13 years. This unique role is an opportunity to join our team as we increase our support of families and children transitioning out of early intervention services and navigating the developmental and educational challenges of the elementary and tween years.
Vision for Case Management Expansion:
Our goal is to create a seamless continuum of care that maintains the relationships, trust, and support built during early intervention. By offering specialized case management for ages 3-13, we provide families with ongoing guidance, advocacy, and access to resources tailored to the needs of elementary children. This approach fills a critical gap in services, focusing on the social, emotional, and educational growth of this age group while encouraging families to navigate transitions confidently.
Responsibilities
- Provide individualized case management services to families, including assessments, goal setting, and regular follow-ups.
- Advocate for children in school settings, including IEP/504 plan meetings and behavioral supports.
- Guide families through transitions, including middle school preparation and accessing extracurricular opportunities.
- Monitor and evaluate the progress of children’s development, adjusting plans as needed.
- Maintain documentation in accordance with agency and state regulations.
Skills
- -Bachelor’s degree in social work, education, psychology, or a related field.
- - Experience working with children aged 3-13 and their families.
- - Strong organizational, communication, and advocacy skills.
- - Familiarity with remote work platforms and willingness to adapt to agency-provided technology.
- - Self-motivated and capable of working independently while meeting accountability standards.
- -Strong interpersonal skills with the ability to build rapport with diverse populations.
