What are the responsibilities and job description for the Supportive Housing Case Manager position at Serenity Mesa Recovery Center?
Serenity Mesa Recovery Center is an adolescent sober living and recovery center in Albuquerque, NM, under the ownership and direction of Healing Addiction in our Community (HAC), a New Mexico non-profit corporation. Serenity Mesa also has a housing component-a shared house, and scattered apartments. This position is primarily for scattered sites, and for overseeing the shared site case manager as well.
The Housing Case Manager Lead will be responsible for providing case management services to participants in our scattered apartments and our single site location, The Village, as well as providing guidance and feedback to the housing case managers.
Essential Job Functions for Housing Participants
o This position reports to the Housing and Navigation Director.
o Work with participants in the Housing Program by assisting them with move-ins and ongoing weekly case management sessions to provide life skills and teaching them how to navigate daily life.
o Complete application paperwork which includes performing the VI-SPDAT and HMIS paperwork.
o Assist participants in locating apartments throughout the city and guiding them through the rental process which includes lease signing with the client.
o Working with property management companies to ensure the client is maintaining tenancy compliance and preventing evictions from occurring.
o Maintain weekly appointments with housing participants at their dwelling which includes apartment inspections.
o Oversee the housing resident’s budget and assisting with finding secure employment.
o Assist housing participants with opening and maintaining a checking and savings account.
o Perform random housing checks to ensure the resident does not have drug paraphernalia or evidence of alcohol consumption and that housing resident is maintaining cleanliness. If cleanliness is not being maintained, assist the resident in identifying ways to improve.
o Perform observed UAs on participants as needed and process UAs to ensure lab orders are submitted and logged properly.
o Establish a service plan which is to be maintained every ninety days. The service plan should consist of learning independent life skills, education, employment, and any other goal identified by each client.
o Complete Casey Life Skills Assessments and update every ninety days.
o Monitor the resident’s medication intake and ensure their medication management appointments are being attended to and assist them when there is trouble with attending their appointments.
o Ensure the participants are utilizing public assistance such as SNAP benefits and health insurance.
o Schedule treatment team meetings with the resident’s housing team as needed.
o Help housing participants establish a positive recovery and support network by exposing them to different methods of treatment and support or any other resource that would benefit the housing client.
o Help housing participants work through triggers, and relapses by providing support and access to resources.
o Enter the proper documentation into EMR Bear after each encounter.
o Act as liaison between Serenity Mesa Housing and property managers to resolve issues such as payment errors, and tenant issues.
o Serve as a buyer for furniture and household items needed to furnish the apartments.
o Report areas of improvement and training opportunities to the Housing and Navigation Director to establish a cohesive program.
Other Miscellaneous Duties
o Assist inhouse case managers with current participants by transporting participants to appointments, meeting with participants to cover vacations or illness.
o Assist inhouse case managers with conducting life skills.
o Any other inhouse case management duties as assigned.
Essential Job Requirements
o Highest education level: High school diploma or equivalent mandatory; some college preferred.
o Certified Peer Support certification preferred.
o At least two years working with a- risk population and mandatory.
o One year minimum of housing case management experience preferred.
o Bilingual a plus (Spanish/ English).
o Ability to pass drug screening and all background checks is essential.
o Ability to pass a motor vehicle record check is essential.
o Fluency in verbal and written English communication is essential.
o Maintaining ethical boundaries
Work Environment
Most duties of this position require being out in the community. It is presumed that the individual in this position will not take advantage of transportation reimbursement by reporting mileage that did not involve the meeting or transportation of housing and aftercare participants, or any other offsite duties required by this position. It is also presumed that the individual in this position uses their hours in the community solely for the purpose of meeting housing participants or any other offsite duties required by this position and will not perform personal matters outside a scheduled lunchbreak. The position requires the ability to work under pressure and with a diverse population, including staff, physicians, participants, patients, insurance companies and other members of the public on a regular basis. The position may occasionally cause exposure to communicable diseases, bodily fluids and other conditions common to a residential environment.
