The Behavior Support Coordinator is pivotal in intertwining person-centered planning, community inclusion, instructional support, and positive behavioral support planning. Key Responsibilities :
Integration of Support Services :
- Develop person-centered strategies, ensuring community integration and positive behavioral support.
- Perform person centered evaluations, write and update positive behavior support plans and ensure thorough documentation as required by State and grant contracts.
- Lead the facility's understanding in positive behavioral supports, trauma-informed care, diversity, equity, inclusion, and other vital practices of Residential Supported Living.
Client Coordination :
Handle client intake / admissions and assessments, as well as transition into and out of the facility.Lead behavior and crisis response initiatives.Providing emotional support directly to the residents.Coordinating with PCPs, case managers, and other staff members at Serenity.Behavior Support Planning :
Assess residents and write individualized Behavior Support Plans.Train staff on these plans, focusing on proactive and reactive strategies based on varied factors influencing the behavior.Monitor the effectiveness of these plans, ensuring necessary reassessment and revisions as needed.Team Collaboration :
Participate actively in service planning teams, ensuring cohesion with care conferences and interdisciplinary team meetings.Serve as a direct connection to DHS Case Managers and the Health Services team.Administration :
Engage in weekly IDG meetings and perform other duties as required.Job Scope :
You will work under the direct guidance of the Executive Director and Program Director, adhering to predefined procedures and guidelines. We are a 25 bed Residential Care Facility facility with complex behavioral and medical needs.
Preferred Skills :
Prior experience in a residential program catering to individuals with developmental disabilities.Experience with crisis cycle and managementTeam player with strong communication skills (both verbal and written in English).Problem-solving expertise and the ability to handle emergency situations calmly and efficiently.Able to use Google Drive, email, calendar and other Google services.Knowledge of state and federal regulations, including OARs.Ability to prioritize and meet deadlines.A friendly approach with a knack for connecting with people.Experience (or a keen interest) in working with super special folks with developmental needs.A cool head in tricky spots and a can-do attitudePreferred Educational and Experience :
Degree in Special Education, Psychology, or Human Services is desired.Prior experience in DD or APD group homesExperience with Positive behavior supports, including writing them, updating them, and training on them.Job Conditions :
