Your Role
As the Intake Specialist, you will conduct mental health assessments and complete treatment plans for new consumers within the Community Mental Health and Counseling (CMHC) department at Sertoma Star Services. The position also maintains a small caseload of consumers who are considered high-risk. The position will have the responsibility of completing crisis assessments in the office and in the community and providing therapy to stabilize consumers in order to transition them to a lower level of care. The Intake Specialist also provides referrals for those not suitable for programming.
Responsibilities
Administer mental health assessments to consumers at intake. Complete all required documentation of services provided in compliance with Rule 132/Rule 140 and agency policy, ensuring that the philosophy of recovery is integrated in all aspects of service planning and delivery.
Follow-up with intakes from referral to starting program to ensure a smooth transition and payment for services by completing registration forms, discussing intakes with Supervisors/Managers of CMHC Programs and team, and communicating information to consumer.
Provide referrals and resources for consumers who are not eligible for the CMHC program. Assist with expanding the resource list for consumers not eligible for Sertoma services.
Maintain relationships with local mental health providers, community psychiatric hospitals and other support services in order to expand services and enhance supports to individuals served.
Document all mental health and crisis assessments conducted according to Rule 132/140 requirements and bill services provided, including case management.
Maintain a caseload of crisis consumers and provide intensive brief therapy onsite and in the community.
Qualifications
Qualified Mental Health Professional (QMHP), with Master’s Degree in counseling, social work, psychology, or related field required.
Knowledge and experience with the documentation and billing process for Medicaid community mental health services preferred.
Effective communication skills, both written and oral.
Valid driver’s license and documentation of current auto insurance, with an acceptable motor vehicle report.
Proficient in the use of computers, software applications, and working knowledge of Microsoft Office Suite programs.
Salary : $50,000 - $55,000
