Shifts: Monday - Friday, must be available to work evenings, weekends, and on call.
Service Alternatives is looking for a Case Manager for a Full-Time position in the Spokane area, supporting at-risk youth in an in-home setting.
Job Summary:
Case coordination and service planning for youth and families experiencing conflict.
Connect youth and families to resources through case coordination, including connections to mental health services, family counseling, educational supports, and other medical, mental, and educational supports
Some Job Duties Include:
- Assessment and Evaluation: Assess clients’ behavioral health needs and develop individualized plans.
- Implementation of treatment plan goal: Support staff teams and placements in achieving the designated outcomes and overall goals outlined in the treatment plans for the youth served.
- Coordination of Care: Coordinate with healthcare professionals, social workers, and other staff.
- Monitoring Progress: Track and adapt clients’ treatment plans as needed.
- Advocacy: Advocate for clients and coordinate with other programs and resources.
- Crisis Intervention: Provide support during urgent situations.
- Documentation: Maintain accurate records of client status and services.
- Collaboration: Work with community organizations, healthcare providers, and government agencies.
- On Call: Will participate in an on-call rotation for weekend coverage
Minimum Qualifications:
- Associate’s Degree and 3 years experience working with youth and families in a setting as outlined below.
Preferred Qualifications
- Bachelor’s Degree in Social Services or a closely related field and 2 years experience working with youth and families in a setting as outlined below.
Exceptional Qualifications.
- Master’s Degree in Social Services or a closely related field and 1year experience working with youth and families in a setting as outlined below.
Acceptable settings include:
- State-licensed in-home or facility-based daycare, professional nanny, School setting, or early learning center
- Mental health hospital, Juvenile rehabilitation facility, detention center, law enforcement, or corrections,
- A child placing agency, residential care program, or as a licensed foster parent, Conducting supervised visits,
- Working directly with children or families in a community service organization.
Licensing and Certifications:
- Currently have or within 30 days of being hired CPR/First Aid license
- Current driver license, car, and car insurance.
- TB Test: Within 7 days of first day of employment, or prior to working on site (whichever comes first)
