Position Overview
TITLE: Medication coordinator
DEPARTMENT: Behavioral Health
REPORTS TO: Program Director
Schedule: Monday - Friday 7:00 am - 3:00 pm
Salary: $40,000/annually
Location: Brooklyn, NY 11206
Essential Job Functions:
- Coordinate pharmacy, medical and psychiatric services for all residents within the facility.
- Complete monthly progress notes for all consumers detailing their progress on their medication management goal.
- Schedule annual physical, dental and eye examinations for all residents.
- Responsible for ensuring that all residents have sufficient medications as prescribed by their attending medical or psychiatric provider.
- Responsible for placing orders to local pharmacies and ensuring receipt of all medications.
- Provide regular entries into program logbook.
- Responsible for tracking and documentation of medication errors.
- Responsible for supervising all new staff in the monitoring of resident’s medications.
- Responsible for administering to the consumers a “self-medication test” to determine their ability to take their prescribed medications independently.
- Responsible for escorting residents to medical and psychiatric appointments, whenever necessary.
- Responsible for notifying treatment programs and resident’s families when the resident is hospitalized.
- Participate in Service plan reviews and clinical meetings.
- Regular attendance in staff meetings and trainings.
- Responsible for providing Program Director and Assistant Program Director with a weekly report summarizing the week’s medical/psychiatric events.

Qualifications
Job Requirements:
- High School Diploma or GED; AMAP Certified
- At least one year of experience in direct clinical work with psychiatrically disabled populations.
- Preferred experience would include previous work with homeless population; work in residential settings and/or community health care, patient education/treatment adherence counseling and group work
- Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
- Bilingual preferred.
