Job Description:
We are actively searching for a dedicated and experienced Licensed Practical Nurse to join our team at a long-term care facility in Niagara Falls, NY. As an LPN in Long-Term Care, you will play a crucial role in providing comprehensive nursing care to patients with chronic illnesses, disabilities, or long-term medical conditions.
Responsibilities:
- Provide direct and indirect patient care in accordance with the established nursing care plan and facility policies.
- Administer medications, treatments, and therapies as prescribed by the attending physician.
- Monitor patient vital signs, assess their physical and mental status, and document any changes or concerns.
- Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans for each resident.
- Assist in the coordination of resident admissions, transfers, and discharges.
- Ensure nursing documentation is accurate, complete, and up-to-date.
- Observe and report any changes in patient condition or behavior.
- Promote and maintain a safe and clean environment for residents and staff.
- Provide emotional support and education to residents and their families.
- Adhere to infection control and safety protocols.
Requirements:
- Valid and active Licensed Practical Nurse (LPN) license in the state of New York.
- Minimum of 1-2 years of experience working as an LPN in a long-term care setting.
- Knowledge of nursing principles, standards, and procedures.
- Ability to assess, plan, implement, and evaluate nursing care.
- Excellent communication and interpersonal skills.
- Strong organizational and time-management abilities.
- Ability to remain calm and focused under pressure.
- Proficient in using electronic medical records (EMR) systems.
