Case Manager Full Time
We are seeking a dedicated Case Manager to join our team at REM Community Services, a part of the Sevita family.
Our team is passionate about providing community-based services for individuals with intellectual and developmental disabilities.
Key Responsibilities:
- Provide case management services, including intake, assessment, crisis intervention, advocacy, referral, and monitoring of families.
- Conduct home visits and office-based services, and accompany individuals being served.
- Coordinate care with outreach workers, health educators, nutritionists, and other service providers to prevent service duplication and ensure high-quality care.
- Plan and assist in obtaining services from third-party providers.
- Develop a plan of care for each individual, including services such as advocacy/case management, independent living skills, training, peer support, physical therapy, and cognitive therapy.
Requirements:
- Bachelor's degree in human services or equivalent education and experience.
- One year of related work experience and knowledge of case management.
- Valid driver's license, registration, and insurance.
