What are the responsibilities and job description for the Clinical Supervisor, Care Management (Medicare) position at SF Health Plan?
Reporting to the Manager of Medicare Care Management, the Clinical Supervisor is responsible for the clinical and administrative oversight of their team, consisting of licensed clinical professionals (Registered Nurse Care Managers, Behavioral Health Care Managers) and non-clinical staff (Care Coordinators). You will work together as a care team who are responsible for a caseload of D-SNP members. You are a clinical lead who will represent the needs of their staff and the members served.
Please note that while SFHP supports a hybrid work environment, you are required to be onsite and in-office a minimum of 4 days per month. This is a hybrid position, based in our Downtown San Francisco office.
Salary: $131,000 to $151,000 annual
WHAT YOU WILL DO:
- Responsible for the clinical and administrative oversight of their multidisciplinary care team who are providing care management for high/moderate/low risk members.
- Assist Director and Manager in unified vision to all Care Management, Medicare staff.
- Support in program strategic development; including creation of workflows, desktop procedures, documents, policies and hiring plans.
- Understand and enforce all applicable regulatory and quality requirements for the dual eligible membership (CMS, DHCS, NCQA, and others).
- Oversee the development of interdisciplinary care plans (ICPs).Participate in Interdisciplinary Care Team meetings.
- Review and approve care team member documentation who are unlicensed/non-clinical.
- Provide lead participation in mock and regulator audits, including case preparation, presentation and follow up with Corrective Action Plans (CAP).
- Build and retain engaged staff by using employee engagement data, tools and methods.
- Audit cases to ensure staff understanding and implementation of program priorities and principles.
- Provide regular feedback to supervisees on their work performance, focusing on both areas of strength and opportunities for improvement.
- Resolve daily personnel and performance management issues.
- Work in cohesion with the other supervisors; provide unified direction and oversight to all team members.
- Ensure that daily activities are being monitored with the team.
WHAT YOU WILL BRING:
- A current State of California RN license to practice without restriction; bachelors degree in nursing required.
- Minimum of 3 years of care management experience within a managed care setting
- Demonstrated leadership experience with 1 to 3 years of experience in a supervisory or team lead role.
- Experience with the D-SNP population.
