What are the responsibilities and job description for the LPN Consultant/Educator position at Shadowfax?
- $1,000.00 Sign-on Bonus to be paid out as follows: $500 after successful completion of the 90-day introductory period, and $500 after 180 days of employment. Former staff must be separated for at least 6 months to be eligible for the sign on bonus.
- Full-time, salaried exempt position, $1,230/week
- Community based medical oversight and staff education
- Must be licensed as a practical nurse in the State of Pennsylvania
- Minimum of 3 years post-licensure experience required
- Basic computer skills required
- Must possess a valid driver’s license and at least 3 years of good driving experience with your license, and must meet our driver eligibility criteria
Essential functions include:
- Oversight of medical care for individuals with a primary diagnosis of ID/A
- Attending staff and individual support plan meetings
- Attending hospital/rehab discharge meetings
- Providing staff training and education
- Triaging calls outside of normal working hours
- Ensuring the health, safety, and welfare of individuals with a primary diagnosis of ID/A
