POSITION SUMMARY: The Social Services Coordinator (SSC) serves a crucial role in addressing the psychosocial needs of patients and their families, working to overcome personal and environmental challenges that may impact their health. They conduct comprehensive social evaluations, collaborate with health care teams to develop patient care plans, and facilitate communication between families, medical professionals, and other agencies for a coordinated approach to patient care. The SSC also provides vital support through documentation of patient evaluations, care plans, and progress reports while assisting in understanding the social and emotional elements affecting a patient's health. They play a key role in offering information and referrals for practical, environmental, and financial assistance, utilizing community resources and acting as a liaison between patients, families, and community agencies.
ESSENTIAL DUTIES:
1. Carries out assessment of social determinants of health and participates in the development of the plan of care.
2. Assists patient and family to understand and follow the medical recommendations made by the providers in order to restore the patient to the optimum social and health adjustment within the patient’s capacity.
3. Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
4. Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
5. Assists providers and clinicians in understanding the significant social and emotional factors related to the patient’s health problems.
6. Provides information and referral services for Shalom patient and family regarding practical and environmental needs.
7. Provides information to patients and families on financial assistance programs and resources.
8. Utilizes appropriate community resources and serves as liaison between patients or families and community agencies.
EDUCATION & QUALIFICATION REQUIREMENTS:
- A master’s degree in social work from an accredited college or university preferred
- Bachelor’s degree in social work required
- At least one (1) year of experience in social work in a health care setting preferred
- Experience with the Medicare/Medicaid program and participation in community agency activities, preferred
- Experience with electronic medical records (EMR), e.g., eClinicalWorks (eCW) preferred
Knowledge, Skill, Ability and Work Difficulty:
- Computer literate (MS Word, eCW, etc.)
- Able to recognize the needs and concerns of people within their cultural framework.
- Works independently in the absence of supervision.
- Self-motivated and flexible to daily patient schedule and/or clinic needs
- Able to provide health care in a sensitive, nonjudgmental manner to persons of all economic levels and cultural backgrounds.
