What are the responsibilities and job description for the Supervisor A - CHANT (Health Services) position at Shelby County Government?
Position Summary
This position is funded by a grant for a duration of time as defined by the grantor.
Works under the direction of assigned Manager providing leadership for Community Health Access and Navigation in Tennessee (CHANT) programmatic activities of assigned personnel to improve client health. Client group includes TennCare recipients, pregnant women, and children/youth less than 21 years of age, including those with special health care needs.
Pay Grade: 51
Salary Commensurate with Experience and Education
Minimum Qualifications
1. 1. Three (3) years of lead or supervisory experience in health care setting; AND
2. 2. Bachelor's degree from an accredited college or university in public health, social work; health administration, nursing, or a closely related field; OR
3. 3. An equivalent combination of related education and/or experience.
4. 4. PROOF OF EDUCATION; TRAINING; AND/OR EXPERIENCE IS REQUIRED.
Duties and Responsibilities
1. Ensures the CHANT program operates in compliance with Shelby County Government standards and policies, contract deliverables, and CHANT Guidelines.
2. Evaluates programmatic processes to identify perceived, potential, or actual problems and implements approaches to address identified problems.
3. Provides leadership and guidance for CHANT assigned staff, reviews assigned personnel case load for equity and capacity.
4. Provides direct and reflective supervision, coaching and /or training for staff, including onboarding activities.
5. Serves as the central referral intake personnel and schedules monthly individual programmatic reviews (i.e., REDcap - Research Electronic Data Capture System) with assigned staff.
6. Ensures documentation is accurate for CHANT billing activities per programmatic mandates and guidelines.
7. Works in collaboration with clinical and administrative staff to maintain patient database integrity to ensure safety of HIPAA related information.
8. Analyzes collected CHANT (REDcap/PTBMIS) data to determine the appropriate and necessary care coordination activities.
9. Monitors care coordination pathway progression for enrolled families to ensure proper and effective navigation activities.
10. Reviews and ensures call center, direct, and community-based outreach referrals are complete and appropriate for the CHANT program.
11. Identifies, establishes, and maintains relationships with internal and external partners.
12. Assigns referrals to staff for care coordination activities and responsible for mailing letters to families for care coordination assignments.
13. Communicates with clinical staff concerning medical records, treatment plans, family service plans, transitional plans, and pharmacy requests.
14. Provides care coordination, home visitation and telephonic services as needed in the event of vacancies and/or limited staff.
15. Leads and participates with groups and meetings for program.
16. Performs other related duties as required or directed.
KSAs
1. Knowledge and understanding of current community-based interventions and policies designed to address social determinates of health.
2. Knowledge and understanding of the principles of health behavior, preventive medicine, and epidemiology.
3. Knowledge of program medical terminology with case management/coordination experience.
4. Ability to supervise assign work to staff and provide training to enhance workforce competencies.
5. Ability to plan, organize and lead the work of others.
6. Ability to communicate clearly and effectively, verbally and in writing to team members, public citizens, partnering agencies and other departments within Shelby County.
7. Ability to gather and enter data, compile information, and prepare technical and non-technical reports.
8. Proficient in Microsoft Office (Word, Excel, PowerPoint) or other similar computer software.
Special Requirements
1. Must possess a valid Tennessee driver license or secure one as of employment date.
2. Must have daily access to a personal operable automobile and meet county requirements for automobile insurance upon date of employment.
3. Must be able to attend meetings and activities on weekends and/or after regular work hours as required.
4. Must be able to travel to locations outside of the office.
