What are the responsibilities and job description for the Clinical Pharmacist- PRN position at Shenandoah Medical Center?
1. Review and prepare medications to be dispensed.
- Review medical records of patients to determine the appropriateness of medication therapy.
- Consults with the healthcare team on medication dosages, side effects, correct administration, etc.
- Ensures correct selection of dosage, form, strength and quantity prepared by a pharmacy technician to be dispensed within the facility.
- Reviews new medication orders via CPOE and transcribes written medication orders when appropriate.
- Prepares and reviews intravenous (IV) admixtures.
- Prepares and dispenses hazardous IV admixtures Maintains hazardous medication inventory levels and outdates.
2. Assists in overseeing the SMC Anticoagulation Clinic.
- Monitors INR results of patients who are referred to the SMC anticoagulation clinic, both in person and over the phone.
- Consults with providers when appropriate according to policies and procedures.
- Documents all therapy changes and prescription refills in the electronic health record.
- Demonstrate excellent customer service to patients.
3. Actively participates in a multidisciplinary team.
- Provide recommendations on medication therapy to other healthcare team members when necessary.
- Assist/educate nursing staff with the medication administration processes.
- Perform discharge counseling and other medication education to patients in the facility.
4. Assists the director of pharmacy in administrative duties.
- Performs daily audit of all controlled substance transactions within the facility.
- Assists in maintaining the hospital drug formulary through the P&T committee.
- Assists in maintaining compliance with the Iowa Board of Pharmacy Rules and Regulations.
- Oversees and maintains compliance with current USP guidelines.
5. Performs other duties as assigned.
