We are hiring for the role of client care coordinator. We are a leading provider of home care services in Cuyahoga County and have recently moved our offices from Cleveland to Independence, Ohio. Summary:
The Client Care Coordinator manages and oversees client care services within the home care agency, ensuring a high-quality and personalized experience for all clients. This role includes managing concierge services, following up with clients, and conducting quality assurance and aide supervisory visits. The Client Care Coordinator will play a critical role in maintaining client satisfaction, ensuring compliance with agency standards, and enhancing the overall client experience.
Key Responsibilities
- Coordinate and manage the daily operations of client care services, ensuring all client needs are met promptly and effectively.
- Conduct initial intakes and home assessments with the client and their families.
- Maintain regular contact with clients and their families to monitor satisfaction and address any concerns or changes in needs.
- Ensure all care plans are up-to-date and accurately reflect the client's current status and preferences.
- Oversee the provision of concierge services, including personalized assistance with scheduling appointments, transportation, and other non-medical support services.
- Develop and maintain relationships with local service providers to ensure high-quality and reliable concierge services for clients.
- Ensure all concierge services are delivered in a timely, professional, and courteous manner.
- Conduct regular quality assurance visits to clients' homes to assess the quality of care being provided and ensure compliance with agency standards.
- Perform supervisory visits to monitor caregiver performance, provide feedback, and offer support and training as needed.
- Document and report findings from quality assurance and supervisory visits, and follow up on any identified issues to ensure prompt resolution.
- Serve as the primary point of contact for clients and their families, providing clear and consistent communication regarding care plans and services.
- Collaborate with caregivers, healthcare professionals, and other agency staff to coordinate and optimize client care.
- Participate in staff meetings and training sessions to stay informed about agency policies, procedures, and best practices.
- Assist with scheduling and coordinating caregiver assignments to ensure adequate coverage and continuity of care.
- Support the agency’s compliance efforts by ensuring all client care activities adhere to relevant regulations and standards.
- Bachelor’s degree in healthcare administration, social work, nursing, or a related field preferred.
- Minimum of 2 years of experience in a homecare or other healthcare setting, with a focus on client care coordination.
- Strong understanding of homecare services, including non-medical and concierge support.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and staff.
- Strong organizational skills and attention to detail, with the ability to manage multiple tasks and priorities.
- Proficiency in using electronic client management and scheduling software.
- Flexibility to work occasional evenings or weekends as needed to meet client needs and conduct supervisory visits.
