Summary
Assists residents and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Works in close collaboration with physicians and other health care personnel in resident evaluation and treatment to further their understanding of significant social and emotional factors underlying resident's health problem.
- Coordinates discharge planning process.
- Helps resident and family through individual or group conferences to understand, accept, and follow medical recommendations.
- Coordinates and actively participates in weekly interdisciplinary care plan meetings.
- Provides service planned to restore resident to optimum social and health adjustment within resident's capacity.
- Prepares resident histories, care plans, and reports.
- Participates in planning for improving health services by interpreting social factors pertinent to development of program.
- Provides general direction and supervision to Social Service Designee (if applicable).
- *Maintain the confidentiality of all resident, staff and organization information.
- Supervisory Responsibilities
- Directly supervises the Social Service Designee (if applicable). Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Education and/or Experience
Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
Certificates, Licenses, Registrations
Licensed Social Worker preferred.
