Job Summary:
We are seeking a compassionate and experienced Registered Nurse to join our team to oversee a (12) twelve bed rehab to home unit. As a RN charge, you will oversee CNA's and therapist role in providing high-quality care and support to patients recovering from illness, injury, or surgery. Your expertise and dedication will contribute to the overall well-being and successful rehabilitation of our patients. This unit is the only unit you will work.
Responsibilities:
- Perform comprehensive assessments of patients' physical, emotional, and psychological needs
- Implement individualized care plans based on patients' specific rehabilitation goals
- Administer medications and treatments as prescribed by physicians
- Monitor vital signs and observe for any changes in patients' condition
- Collaborate with interdisciplinary healthcare team members to coordinate and provide holistic care
- Educate patients and their families on post-rehabilitation care, including self-care techniques and home exercises
- Assist with discharge planning to ensure a smooth transition from the rehabilitation facility to the patient's home or another care setting
- Provide support and guidance to patients in managing activities of daily living (ADLs)
- Perform specialized procedures such as post surgical assessments, wound assessments ostomy care, suture removal, IV's and other clinical duties. Able to work closely with our therapist to help patients back to their baseline to return home.
Qualifications:
- Valid Registered Nurse (RN) license in the state of Missouri
- Minimum of 2 years of nursing experience in a rehabilitation setting preferred
- Strong knowledge of rehabilitation nursing principles and practices
- Proficient in medication administration and monitoring vital signs
- Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare team members
- Ability to prioritize tasks and manage time effectively in a fast-paced environment
- Experience with discharge planning and coordination of care for senior patients is a plus
