What are the responsibilities and job description for the Advanced Practice Professional position at Shore Medical Center?
Shore Physicians Group is currently seeking an Advanced Practice Professional with a valid NJ State Board P.A. or A.P.N. license to provide Out-Patient Primary Care.
Our Advanced Practice Professionals function independently collaborating with SPG Physicians to conduct comprehensive examinations, diagnose and treat illness, compile medical data, administer and order diagnostic tests and interpret test results.
Responsibilities
- Provide patient care in one or more of the out-patient practice sites
- Develop, implement and maintain an organized system of patient care
- Work in collaboration with practice physicians to provide multidisciplinary care of patients
- Conduct comprehensive examination, diagnosis and treat illness of patients as delegated by the collaborating physician
- Prescribe, reissue, and adjust dose of medications as delegated by supervising physician and within the scope of State Law
- Administer and order diagnostic test and interpret test results
- Develop and implement patient management plans, record progress notes, and assist in continuity of care
- Compile patient medical data within the medical record
- Consult and/or collaborate with other health care providers
- Interpret test results for deviations from normal
- Perform therapeutic procedures, such as injections, immunizations, and managing infection
- Serve as a resource and positive role model to staff
- Maintain confidentiality
- Maintain knowledge of current relevant regulatory and corporate compliance standards
Requirements
- Advanced clinical degree
- Valid NJ State Board P.A. or A.P.N. license
- Experience in management of patient care and chronic disease
- Recent patient clinical management skills
- Excellent communication skills including active listening, verbal and written communication
- Experience and comfort working with physicians one-on-one and in small groups, including discussing clinical data and evidence-based treatment plans
