What are the responsibilities and job description for the MDS Coordinator/Case Manager position at Sienna Hills Nursing and Rehabilitation INC?
We are looking for an experienced Nursing Facility ADON/MDS nurse to join our team. Applicant must have current Ohio Registered Nurse license.

Essential Responsibilities:
· Must demonstrate the knowledge and skills necessary to complete required MDS's and care plans as appropriate for the residents we serve
· Must be knowledgeable of nursing, medical, and therapy practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities
· Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for completing MDS's
· Must be able to relate information concerning a resident’s condition and assess and address as needed.
Qualifications:
- Valid RN License
- May consider LPN with related experience
- 1 years of experience as a supervisor in a nursing care facility, or other related health care facility with MDS experience
