Shift : Nights, 6 : 45pm - 7 : 15am

Job Description : PATIENT POPULATION :

The patient population served can be all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY :

Responsible directly to the assigned Unit Director. Under general supervision of the assigned R.N. Preceptor, the newly licensed R.N. is assigned patient care activities based upon the individual's learning foundation and their status as a novice.

Under the direct supervision of the R.N. Preceptor, the Novice R.N is responsible for the identification of needs, treatment or care appropriate to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patient's needs. Possesses knowledge of growth and development and has an understanding of the range of treatment needed for these patient groups.

The New Graduate / Novice R.N., under the direct supervision of a qualified R.N. Preceptor, may perform any function previously learned which may include but is not limited to, total body assessments and medication administration. The Novice R.N. will not be assigned individual nursing responsibilities and functions which are beyond the measured competence of their abilities. Responsibilities will be commensurate with the New Graduate's level of competence and within the policies at Sierra View Medical Center (SVMC).

Must be able to work normal / scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

To perform this job successfully, an individual should have successful completion of orientation to area which assigned. Must be currently licensed as a New Graduate R.N. in California. Must have and maintain current BLS certification within orientation process, prior to hands-on patient care.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required.

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

To perform this job successfully, an individual must have basic computer skills.

LICENSURE / CERTIFICATIONS :

Must be currently licensed as a New Graduate R.N. in California. Must have and maintain current BLS certification within the orientation process, prior to hands-on patient care.

Responsibilities and Essential Functions :

  • Indicates Essential Function
  • 1 Demonstrates ability and flexibility to assist in unusual clinical situations, and assigned tasks, i.e. changes in patient acuity / reassignment, unscheduled admission, life-threatening patient situations.

  • Communicates essential information to Director, preceptor and / or Charge Nurse, reporting significant changes in patient status.
  • a Demonstrates sensitivity and awareness of patient's rights in handling confidential patient information.

    3 Demonstrates an understanding of the use, actions and interactions of drugs used in combination for patients with multi-system problems.

  • Demonstrates adherence to the Joint Commission "National Patient Safety Goals for Hospitals" as defined by scope of practice.
  • 5 Documentation reflects compliance with established hospital policies and procedures.

  • Be in attendance on site and arrive to work on time.
  • CLINICAL :

    10 Delivers patient care with compassion, empathy and respect utilizing the nursing process model.

    a. Needs identification :

  • Assists in obtaining general patient information.
  • Encourages patient, family, significant other participation in identification of needs, i.e. physical, emotional, educational, discharge, etc.
  • Under direct supervision of the R.N. Preceptor, identifies pertinent patients' needs / problems.
  • Is responsible for the treatment of care appropriate to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patient's needs. Possesses knowledge of growth and development and has an understanding of the range of treatment needed for all patient groups.
  • b. Plan :

  • Under direct supervision of the R.N. Preceptor participates in ongoing revision of the individualized patient care plan identifying measurable long / short - term goals and discharge plans.
  • Identifies and utilizes appropriate resources, i.e. chaplain, dietician, etc.
  • c. Implementation under direction of R.N. Preceptor :

  • Attempts to ensure patient understanding of special preparation for procedures performed in and by other departments to facilitate a coordinated effort toward optimal patient care.
  • Delivers basic nursing care utilizing policies, procedures and standards of care.
  • Safely performs administration of medication.
  • Accurately reports and documents assessments, patient data, medication, treatment, procedures and contributes to evaluation of progress towards goals.
  • d. Evaluation under direction of R.N. Preceptor :

  • Evaluates patient response to specific nursing interventions.
  • Under the direct supervision of the R.N. Preceptor reviews the plan of care of each shift and provides input to R.N. Preceptor for updating and changing the plan as indicated by patient needs and response.
