Job Summary
We are seeking a dedicated and compassionate Home Health Physical Therapist Assistant to join our healthcare team. In this role, you will work under the supervision of licensed physical therapists to assist patients in their rehabilitation journey. Your primary focus will be on implementing care plans, providing therapeutic exercises, and ensuring that patients achieve their rehabilitation goals.
Duties
- Assist in the implementation of individualized care plans as directed by the supervising physical therapist.
- Provide hands-on therapeutic interventions, including exercises and modalities to enhance patient recovery.
- Monitor patient progress and report any changes or concerns to the supervising therapist.
- Educate patients and their families on home exercise programs and strategies for continued improvement.
- Maintain accurate documentation of patient treatments and progress notes in compliance with regulatory standards.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Participate in discharge planning to facilitate smooth transitions for patients returning home or moving to other care settings.
- Support patients in various settings such as outpatient clinics, home health environments, and acute care facilities.
Experience
- Previous experience as a Physical Therapist Assistant is preferred but not required; new graduates are encouraged to apply.
- Familiarity with various patient populations, including geriatrics, is beneficial.
- Experience in specific areas such as orthopedics, acute care, or outpatient therapy is a plus.
- Strong communication skills and the ability to work collaboratively within a healthcare team are necessary for success in this role.
