Position Overview:
SIHF Healthcare is seeking a full time Nurse Practitioner, dually boarded in both psychiatric mental health and family medicine to support SIHF Alton School-Based Health Center. This role will provide high-quality, patient-centered care in an outpatient school-based setting, to school-aged children and faculty members. Working collaboratively with school staff and other medical support personnel to ensure the delivery of accessible and compassionate care. This position requires a clinician who shares our values and mission, committed to delivering care that addresses the whole person and contributes to improved health outcomes.

As part of our commitment to patient-centered care, the clinician will collaborate with interdisciplinary teams to provide continuous care and contribute to the overall wellness of patients.

Key Responsibilities:

• Provide direct clinical services, including assessment, diagnosis, and treatment for students and faculty in an outpatient school-based health setting.
• Utilize telehealth services as appropriate to provide remote consultations and follow-up care for students and faculty who are unable to attend in-person appointments.
• Collaborate with school physicians, specialists, and other healthcare providers in managing patient care, referring students and faculty as necessary for specialized services.
• Participate in school outreach programs to promote health education, disease prevention, and health awareness.
• Ensure compliance with healthcare policies, procedures, and regulations, including those specific to school-based health programs and HIPAA guidelines.
• Attend school-related events that may be offsite or at our different school-based centers to provide health support and promote wellness initiatives.
• Maintain accurate and timely documentation of patient care, adhering to all legal and regulatory standards.
• Must adhere to yearly quality assurance metrics to ensure optimal care and performance standards are met.
• Ensure high levels of proficiency within scope, following the expectations of the Patient-Centered Medical Home model.

Qualifications:

• Illinois Nurse Practitioner licensure or eligibility for licensure in the state of Illinois.
• Master’s degree in nursing required.
• Family Nurse Practitioner (FNP) certification is required.
• Psychiatric Mental Health Nurse Practitioner (PMHNP) certification is required.
• Two plus years of experience as a Nurse Practitioner is preferred.
• Knowledge of the Patient-Centered Medical Home (PCMH) model and a commitment to its principles.
• Fluency in English and Spanish preferred, not required.
• Strong interpersonal skills and ability to work collaboratively in a team-based environment.
• Excellent communication skills, both verbal and written.
• Ability to maintain confidentiality and adhere to ethical and legal guidelines in clinical practice.
• Must have reliable transportation and a valid driver’s license.
