The Social Worker provides direct clinical support to patients and is involved in the therapeutic process from admission to discharge, providing individual, family and group therapy and preparing discharge plans as indicated. The Social Worker interfaces with internal and external stakeholders to ensure patient-centered, evidence-based, high-quality care for patients. Social Workers who perform in a per diem capacity only lead clinical groups.
Please note: This is a temporary, 6-month position.
Duties/Responsibilities:
- Carries a clinical caseload, conducts family therapeutic meetings, utilizing biological, psychological, and systemic perspectives in understanding patients and their families to develop treatment plans and interventions from those perspectives and ensures that care is appropriate to the ages of patients served.
- Actively participates in clinical rounds & social work staff meetings and training.
- Facilitates discharge planning and implementation.
- Provides intern &/or staff supervision, as requested by the Director of Social Work or Director of Clinical Operations.
- Conducts group therapy sessions with techniques appropriate to the ages of the patients served.
Required Skills/Abilities:
- Must have the ability to function optimally in a stressful environment, and the ability to remain calm in emotionally charged situations.
- CPR BLS Certification required
- Nonviolent Crisis Prevention certificate required
- Proficiency in Microsoft Office suite
Education and Experience:
- Master’s degree in social work or equivalent degree in related field
- Licensed Master Social Worker by the state of Connecticut
- Licensed Master Social Worker by the state of New York
Physical Requirements:
- Prolonged periods sitting at a desk and working on a computer.
- Walking to common areas for group sessions
