What are the responsibilities and job description for the OVERNIGHT PHARMACIST position at Sinai Chicago?
GENERAL SUMMARY/BASIC PURPOSE OF JOB:
Responsible for providing pharmaceutical care to patients including appropriateness, safety, and
efficacy of therapy for HIV and HCV infections; Reviews patient medication lists; Monitors for drug
interactions; Communicates and works with patients on understanding the importance of adherence
to medications; Ensures and verifies that medications are ordered appropriately; Works with outside
specialty pharmacies to ensure medications will be obtained through prior approval process for HCV
and HIV therapy; Assist with coordination of communication between physicians and patient
navigators; Provides ongoing assistance in the orientation and training of Infectious Diseases
physicians, residents and students; ongoing training for support staff on medications used for
treatment of HIV and Hepatitis C; Keeps the Pharmacy Operations Manager and Director of Pharmacy
appraised of daily activities of the ID division and clinic; Duties include working to provide quality
patient care and serve as a medication expert for HCV and HIV infections.
MINIMUM EDUCATION:
ï‚· Graduate of a college of Pharmacy accredited by the American Council on Pharmaceutical
Education (ACPE). Doctor of Pharmacy and hospital pharmacy residency preferred.
MINIMUM WORK EXPERIENCE:
ï‚· One year of hospital pharmacy experience preferred.
KNOWLEDGE & SKILLS:
ï‚· Excellent drug knowledge base including pharmacokinetics, dosing and nutritional support.
ï‚· Good computer skills.
ï‚· Effective verbal and written communication skills.
REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS:
ï‚· Licensed as a Registered Pharmacist in the State of Illinois.
