The Case Manager for Accessibility Services is responsible for providing case management, community resource connection, substance abuse referral and support. These services are provided through Care Team report and referral from the Basic Needs grant counselor or program director. The Case Manager will conduct outreach and referral for students referred to the care team for intervention as well as provide linkages to students needing additional basic needs resources referred by the basic needs counselor and program director.

Principal Accountabilities
- Case management of students referred to CARE Team and/or seeking Supported Education Counseling
- Identification of off-campus community resources and asset mapping
- Referral to on-campus and off-campus resources
- Data collection and retention and workshops to students. These services are provided by appointment or on a walk-in basis and pertain to a wide range of needs including basic needs support, personal/emotional, academic and career issues
- Provide assessment, resource connection, evaluations and referrals to college departments, as well as external resources that promote student development through goal achievement
- Reports from open to close to ensure that students are being served in an effective and expedient manner
- Develop and maintain comprehensive community and college resources for students to access in office
- Link to providers and assist in applying or referring for services
- Strong customer service and/or student advocacy as demonstrated through a positive attitude of approachability, adaptability, strong problem-solving and desire to identify and support student success
- Engage in behaviors that create an inclusive environment in which all people are valued and supported
- Goes beyond the first or simplest response to a customer, working to identify the root cause of the problem, question, or issue to resolve it efficiently and professionally
Requirements
- Licensed social worker with minimum of a bachelor’s degree in social work; master’s degree preferred
- Minimum of three years of social work experience required
- Excellent verbal and written communication skills and demonstrated evidence of required customer skills
- Excellent listening and problem-solving skills required
- Skills to assess students’ needs and concerns and interpret appropriate relevant academic reports to determine students’ strengths and identify resources to resolve problems required
- Broad knowledge of human development and a variety of student concerns and social problems, including relationships, substance abuse, parenting and domestic violence preferred
- Crisis intervention knowledge and skills preferred
- Knowledge about career, personal interests, and learning styles preferred
- Knowledge of community, community colleges and, specifically, knowledge of Sinclair Community College processes and procedures to advocate and make effective referrals preferred
- Ability to be self-directed, self-motivated, solutions and win-win oriented preferred
- Organizational and presentation skills, including workshop or program planning skills preferred
