Job Summary
The Patient Navigator performs a variety of duties which may include, but are not limited to: Enhancing the patient experience through facilitation/coordination of care, informing and educating patient on community resources and striving to improve the “global” patient experience. Works to keep patients engaged with services, address issues related to relapse, and communicate with the team on patient needs. Works effectively and cooperatively within the workplace. Essential Functions
Works closely and collaboratively with Hub & Spoke Care Navigators to provide coordinated care and to increase low-barrier access to medication assisted treatment and recovery support services Facilitates ongoing coordination of follow-up care to treatment and recovery support services (e.g., transportation, housing, employment, mental health and/or chemical dependency assessments, primary care, food and clothing resources) offered at the Hub and Spoke sites and various partnered agencies Performs individual patient screening to determine needs and lacking resources Provides medication assisted treatment (MAT) education for patients Performs patient charting in the EMR system Provides support for individual’s self-management and engagement Performs community outreach efforts to various agencies, as well as underserved communities Complies with national Protected Health Information laws; this includes HIPAA and 42 CFR Part 2 Take responsibility for professional development to maintain and increase competency and support the education of our patients, families and staff by: - Participating in ongoing education relevant to role. - Reaching out, establishing and maintaining relationships with community resources to develop referral resources and patient advocacy/support networks. Maintain confidentiality of patient information and medical records. Communicate effectively and cooperatively in the workplace. Actively contribute to the achievement of Quality Improvement and Lean Process Improvement activities. Complete all mandatory learning requirements in a timely manner. Attend and actively participate in all department, team and/or committee meetings. Comply with all Skagit Regional Health, department and location specific policies and procedures. Ability to travel to different clinics to assist in team training on an as needed basis. Maintain ability to effectively utilize Microsoft Office (Outlook) and department specific software applications to perform work. Perform other duties as assigned by Supervisor or designee.
Education
AA required within 3 years of hire; BA strongly preferred.
Experience/Training
Two years’ experience working in a hospital or healthcare setting, preferred. Practical working experience as a Patient Navigator strongly preferred. Preferred minimum two years of experience in facilitation of patient care services.
License/Certifications
Patient Navigation Certification preferred. Certification or licensed as a licensed chemical dependency profession (CDP), behavioral healthcare worker, social worker or primary healthcare worker, preferred.

While performing the duties of this job, the employee is regularly required to sit for long periods of time; when working in office. Repetitive tasks such as typing, sitting, answering phones, and interacting with computers and computer systems most of the day is a function of the position. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to focus.
