We are currently seeking an experienced and dedicated Director of Social Services to lead our social services team and ensure the well-being of our residents.
Position Summary :
The Director of Social Services will oversee and manage the social services department within our Skilled Nursing Facility. This is a leadership position responsible for developing, implementing, and supervising social service programs for our long-term care and short-term rehabilitation residents.
Responsibilities :
- Manage all aspects of the Social Services department, ensuring compliance with federal, state, and local regulations.
- Supervise, guide, and support Social Services staff to maintain high standards of care.
- Conduct assessments and develop care plans to address residents' physical, emotional, and social needs.
- Implement therapeutic programs and interventions for residents.
- Collaborate with the interdisciplinary care team to provide person-centered care to residents.
- Advocate for residents' rights and serve as a liaison between residents, families, and staff.
- Ensure accurate documentation and maintain records according to regulatory standards.
- Provide crisis intervention, counseling, and support for residents and their families.
- Participate in care conferences and interdisciplinary meetings related to resident care.
