Key Responsibilities:
- Complete MDS assessments in accordance with federal and state regulations and guidelines. Ensure accuracy and completeness of data.
- Resident Assessment: Conduct comprehensive assessments of residents, including physical, psychological, and social aspects. Utilize the MDS tool to capture resident status.
- Care Planning: Develop and update individualized care plans based on MDS assessment data. Coordinate care plan meetings with the interdisciplinary team.
- Regulatory Compliance: Ensure compliance with all applicable regulations and standards, including CMS guidelines and state-specific requirements.
- Interdisciplinary Collaboration: Work closely with other healthcare professionals, including physicians, therapists, social workers, and dietitians
Requirements:
- State licensure as a Registered Nurse (RN)
- 2 years of experience in a healthcare facility or related setting
- Strong understanding of skilled nursing standards of care
- Excellent critical thinking and communication skills
