Job Summary
We are currently looking to hire Case Manger to cover areas Columbia, Irmo, Lexington, Orangeburg, Newberry, Swansea
If you are looking to set your own schedule Monday-Friday from 7am-7pm, and willing to build a case load and travel to homes for visits every quarter, annually (re-evaluation) initial visit and monthly phone calls. Then this is the career change you have been looking for. We offer part-time and full-time employment. No benefits offered. $30 per client, monthly (phone contact) $41 per client for home visit (quarterly, initial, re-evaluation)
The case manager is responsible for providing coordination, monitoring, and evaluating treatment plans and their progress; facilitating interdisciplinary approaches and monitoring progress for participants covered under Medicaid. Work closely with family, friends of the participant, physicians, nurses, mental health and other health care professionals, as well as maintain the participant’s electronic file. Make appropriate referrals for the participants to obtain needed services. Extensive documentation is necessary to ensure that the individual receives needed services and progress towards goals. Collaborates with service providers to obtain services for the participants. Implements and coordinates all case management. Activities relating to Medicaid cases in compliance with regulatory and company policies and procedures. You must make an in-person home visit and use community resources
Qualifications

Ability to analyze complex situations and recommend appropriate courses of action
Strong verbal and written communication skills. Must be able to write cohesive and comprehensive documentation.
Ability to handle confidential and sensitive information professionally with discretion and integrity.
Excellent organizational and time management skills
Ability to establish and maintain effective working relationships with clients, staff, & community members
Ability to work independently and collaborate with others.
Ability to organize and conduct several tasks and project simultaneously.
Ability to handle conflicts
Ability to travel within designated geographic areas for in-person visits
Effective computer skills include navigating multiple systems and keyboarding.
Additional Requirements:
Must pass a National Criminal Background Check and a sled background check
Must have a valid driver’s license and produce 20 years of driving record
Must maintain cell phone service (smartphone)
Must demonstrate skills in computer usage
Must attend a week and ½ training for new Case Managers and pass a entry state test
Must have or be willing to obtain an updated TB test
Required education: BA/BS degree or certification
Must be willing to travel. Travel 50%
Must be willing to build a caseload
