The Position : The Nurse Practitioner occupying this role with Snowline ElderCare will deliver comprehensive, person-centered primary and palliative care services to seniors in home-based clinical settings. This position emphasizes quality of life, health promotion, and chronic disease management, aligning care delivery with the mission and values of Snowline Health as well as the local community. The provider will work collaboratively with the multidisciplinary team to address complex health needs, coordinate care, and ensure patient and family satisfaction. The providers in this role will play a key role in care plan development and optimizing transitions of care for program enrollees.

Responsibilities : As a Nurse Practitioner (NP) at Snowline Health, you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families. The position is based out of Diamond Springs, CA, and involves a hybrid of on-site and remote work. The role focuses on providing comprehensive, patient-centered primary and palliative care to elderly patients in home-based and clinic settings.
Your duties will include :
Clinical Care providers in this role will confidently be able to deliver in residence care and develop comprehensive care plans for complex patients with team coordination.
