What are the responsibilities and job description for the Client Care Coordinator Substance Abuse position at Soba New Jersey?
Responsibilities:
Admissions Functions:
- Obtains release for referring agency, and reiterates to client from point of intake discharge planning to this facility
- Notifies referring agency that client has arrived
- Send clinical documentation within 48 hours of client’s admission
- Maintains contact with referring agency regarding client updates, scheduling ambassador visits, conducting pre-assessments, and planning for transfer
- Verification of Insurance benefits
Clinical Functions:
- Greets new unattached clients (during business hours) and provides them an overview of the program and continuum of care offered at SOBA
- Completes Client Care Assessments as a part of client’s admissions process
- Brief daily check in with clinical director regarding all client’s aftercare plans
- Finds appropriate referrals for clients unable to remain in SOBA’s continuum (due to employment/childcare/financial/proximity reasons) and sets up aftercare appointments
- Obtains and completes FMLA paperwork for all clients requesting this, liaising with clinical and medical team
- Obtains release for any legal entities and provides regular updates
- Assists with completing clinical intakes/transfers, as needed
- Group facilitation
General Functions:
- Ensures correct information on Face Sheet and Releases, including family contacts, referral agents, legal entities, etc.
- Documents all client interaction in clients’ charts, including discharge planning efforts, both for referral sources and unattached clients
- Communicates regularly with admissions and clinical staff
- Attends weekly Group Clinical Supervision and MDT meetings
Qualifications:
- Must be able to communicate with clients and staff members through various forms of communication, including email, phone and text
- Computer literacy is required. Must be able to use Word, Excel, etc. Experience on Kipu a plus.
- Bachelor’s degree in related field is preferred.
- NJ CADC is preferred.
- 3 years of experience working within a drug or alcohol facility.
