Family Medical Case Manager needed for full-time, temporary (4 / 6 weeks) in Philadelphia County.
Based in West Philadelphia with two days per week at Children's Hospital, this role provides comprehensive HIV case management for individuals and families. Responsibilities include assessing client needs, developing and monitoring care plans, linking clients to medical and support services, coordinating care, advocating for entitlements, and conducting home visits as needed. The case manager will collaborate with service providers, offer education and referrals, and ensure timely documentation.
Position Requirements
BSW or MSW with experience in social / human services. Strong case management, client advocacy, and women's health expertise preferred. Some supervisory experience is a plus.
