As part of the Direct Services team, they assess client needs, develop and monitor care plans, coordinate services, and advocate for entitlements. Responsibilities include accompanying clients to medical visits, conducting home visits, collaborating with service providers, and offering education and referrals. They may provide on-site case management in clinics or hospitals and serve as a resource on AIDS-related issues. Timely documentation, participation in staff meetings, intake coverage, and other duties as assigned are required.
