Responsibilities include accompanying clients to medical visits, conducting home visits, collaborating with service providers, and offering education and referrals. They may provide on-site case management in clinics or hospitals and serve as a resource on AIDS-related issues. Timely documentation, participation in staff meetings, intake coverage, and other duties as assigned are required.
