In This Dual Role, You Will:
- Conduct in-home psychosocial assessments for Medicaid recipients receiving in-home services
- Coordinate care plans, connect individuals and families with critical resources, and ensure ongoing support
- Provide crisis intervention, supportive counseling, and advanced care planning
- Serve as a liaison between clients, families, agencies, and the agency
- Educate patients and families on available resources and advanced care directives
- Participate in interdisciplinary meetings
- Maintain thorough documentation to ensure regulatory compliance and high-quality care
