We are seeking a dedicated Medical Hospice Social Worker for a full-time or part-time temp-to-hire position in New Castle County. In this role, you will be a vital part of a multidisciplinary healthcare team, providing skilled support to clients in need of nursing and rehabilitative care within their homes. Your primary responsibility will be to assess and address the social and emotional factors affecting clients' health while facilitating access to necessary community resources.
Position Requirements
DE LMSW and have at least two years of community-based experience. Prior home care experience is preferred, along with strong computer skills and familiarity with electronic medical records. The role requires reliable transportation for travel to assigned cases.
