SUMMARY : Lead and direct the overall social service function of the Center in accordance with residents’ needs, government regulations, and company policies so as to maintain each resident at their maximum potential.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Plan, develop, organize, implement, evaluate, supervise, and direct the social services programs and activities.
Ensure an on-going CQI program and develop and implement appropriate plans to correct deficiencies.
Consult with and coordinate social services activities with administration, medical and nursing services, Consultants, and other related departments and implement changes as required.
Ensure that necessary forms, reports, and progress notes are properly completed and contain informative descriptions of the services provided and the residents’ responses to the services.
Participate in community planning, development, and implementation of social care plans, resident assessments, and discharge planning to meet the needs of the center, the residents, and their families.
Coordinate social services programs with the residents, their families, staff members, and community agencies through interviews, consultations, and direct involvement in the programs.
Ensure compliance with employment laws and company policies.
Ensure management practices that maintain high morale and staff retention to include effective communication, prompt problem resolution, positive supervisory practice, and maintaining a positive work environment.
Manage turnover and ensure future staffing through development of recruiting sources and appropriate selection and orientation.
Ensure timely preparation of associate evaluations on personnel. Meet with social services personnel on a regularly scheduled basis and solicit input from interdepartmental supervisors concerning the operation of the social services department.
Assist in developing, for each resident, a preliminary and comprehensive assessment and written care plan that identifies the emotional and social problems and / or needs of the resident and the goals accomplished for each problem and / or need identified.
Encourage residents and families, where appropriate, to participate in the development and review of care plans.
Participate in assessing, reviewing, and revising care plans as required, but at least quarterly. Ensure all social services personnel are aware of the care plans and monitor their followthrough.
Ensure a performance improvement program is implemented to monitor and improve the quality of social services delivered.
Maintain resident confidentially.
Know and comply with residents’ rights regulations.
Ensure the residents’ emotional and social needs are met.
Review and promptly report to the Administrator and maintain a written record of all resident complaints and grievances.
Coordinate social services with all other resident care services to assure a smooth integration to quality resident care.
Maintain a working knowledge of applicable government regulations and ensure compliance.
QUALIFICATIONS :
Must be a licensed social worker in the state employed. Must have equivalent social work experience (applicable to centers holding 121 beds or less).
Must have a minimum of one (1) year of related experience in a healthcare setting.
Completion of post-secondary education and be able to read, write, and follow oral and written directions.
Must be capable of maintaining regular attendance as required.
Must meet all local health regulations and pass post-employment exam, if required.
Related social work experience at a level necessary to accomplish this position.
PHYSICAL DEMANDS AND ENVIRONMENT :
Frequent walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing, and pulling.
Willing to work beyond normal working hours and during disastrous circumstances, if needed.
