Description
The Chronic Care Management Team (CCM) is seeking a fulltime energetic individual who is caring, responsible, and dedicated to providing patients with an exceptional healthcare experience. This Patient Navigator should have a strong knowledge of medical terminology and also a basic knowledge of healthcare plans and insurance verification. Excellent communication skills, both verbal and written, is a must have. This position requires a positive attitude and personable demeanor. It also requires great time management and organizational skills with special attention to detail. Responsibilities include but are limited to the items below:
· Create ongoing relationships with our patients and family members and advocate for their health and respond to messages left by patients.
· Functions as a liaison between patients, physicians, staff, and outside medical offices to improve communication and patient care.
· Initiates, monitors, and coordinates care planning for assigned patients, including patients receiving immunotherapy infusions Communicates effectively and maintains a professional relationship with co-workers and providers.
· Verify patient’s insurance coverage and eligibility for CCM care and accurately submit prior authorizations to insurance plans in a timely manner.
· Follow up and troubleshoot any pending authorizations, on a regular basis, to obtain the current status or to be informed of any action needed for approval.
· Prepares documentation for billing purposes for the care navigation program. Maintain proper documentation in EHR for continuity of care.
· Ability to successfully navigate between multiple systems throughout the course of the workday, this includes but is not limited to operational software and vendor portals.
· Maintains confidentiality according to HIPPA regulations and AAU policies.
Job Type: Full-time
Salary: $17.00 - $19.00 per hour

- Medical experience: 1-2 years required

Requirements
Min. Education
- High School Diploma or GED, Required
- Associate or Bachelor’s Degree; Strongly Preferred.
Min Experience
- 2 Years Medical experience; Required
- 1 Year customer service/call center experience, Strongly Preferred
- 2 Years Oncology and/or Urology experience, Preferred
- 1 Year of navigator and/or educator experience; Preferred
