Position: Healthcare Navigator
Position Information: Full-Time Salary
Organizational Summary: Soldier On is a private non-profit organization providing services for veterans that are homeless or at risk of becoming homeless. Soldier On operates over 120 beds of transitional and emergency beds in separate facilities for male and female veterans in Western Massachusetts. In 2010, Soldier On began developing permanent housing for veterans in Massachusetts and has over 300 permanent housing units in the state. Soldier On has partnered with developers to provide case management services in 36 units of permanent housing in upstate New York and has been awarded funding through the Empire State Supportive Housing Initiative (ESSHI). In New Jersey, Soldier On operates over 70 units of permanent housing. Soldier On has been the recipient of three SSVF grants since September 2011. The SSVF program is a grant funded by the VA awarded to private non-profit organizations and consumer cooperatives. These grants provide case management services, outreach services, housing assistance, healthcare navigation, and temporary financial assistance for veterans that are homeless or at risk of becoming homeless.
Position Summary:
The position provides healthcare coordination, health education, interdisciplinary collaboration, coordination and consultation, and administrative duties. The position is responsible for working in collaboration with the referring case manager and veteran client with the goal of services being long-term independent housing stability. The incumbent will act as a liaison between Soldier On and the VA or community medical provider and works with a population of veterans with complex needs who require assistance accessing health care services or adhering to healthcare plans ensuring accessibility equity and identifying and addressing system challenges. The majority of the job activities are performed in the community; at shelters, social service agencies, veteran’s apartments, and other locations deemed appropriate through a mobile case management model. A company vehicle is provided. The primary catchment area for this position throughout Salem, Cumberland, Cape May and Atlantic Counties. The position reports to the Field Supervisor of Healthcare Navigation.
Qualifications:
· Valid driver’s license required.
· Ability to drive up to 6 hours per day if necessary.
· Relevant education and/or experience in human services, healthcare, or related field.
· Bachelor’s degree required, Advanced degree preferred.
· Advanced understanding of the VA or other healthcare systems
· Advanced knowledge of health insurance specifically Medicare and Medicaid
· Knowledge and understanding of how to access community resources.
· Ability to be supportive and respectful to all veterans and household members served.
· Ability to work in a variety of settings with culturally diverse families.
· Ability to be culturally sensitive and appropriate.
· Capability to work both independently and as part of a team.
· Ability to collaborate with in-house and external referral sources.
· Possess strong organizational, written and oral communication skills.
· Demonstrate a strong level of understanding and comfort with various forms of technology.
· Ability to take direction and participate in weekly supervision.
· Preference given to candidates who are veterans and/or have lived experience with homelessness or housing instability.
Responsibilities:
· Meet with veterans in the community to complete healthcare navigation assessments, applications and other forms.
· Work collaboratively with veterans and their families to develop a healthcare navigation plan.
· Assist with scheduling of medical, mental health and other health related appointments.
· Identify transportation options for the veteran if needed.
· Build and maintain professional relationships with the VA and community organizations, local medical providers, and other relevant stakeholders.
· Document and maintain accurate information in client files, including progress notes on all home visits and contact, on a regular basis.
· Develop and maintain relationships with community service providers.
· Conduct outreach in a variety of settings including, on the streets, in encampments, shelters, hotels/motels, and other places not meant for human habitation.
· Provide timely case updates to the referring case manager.
· Maintain client confidentiality.
· Provide support services in crisis situations appropriately and quickly.
· Document and maintain accurate information in electronic client files as part of the referral process.
· Knowledge and understanding of company guidelines.
· Adhere to agency policy, procedures, and professional code of ethics.
· And other duties assigned by supervisor.
