Job Title : Licensed Practical Nurse (LPN) - Full Time Nights / Weekends

Guaranteed Hours : 40 hours per week
Experience : Minimum 1 year as an LPN
License : LPN License
Certifications : Basic Life Support (BLS)
Must-Have : Experience in a Long-Term Care Facility
Job Description :
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Maintain collaborative relationships with nursing staff, physicians, and ancillary staff.
- Ensure knowledge and clinical expertise through continuing education and in-service programs.
- Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to developing an age-appropriate and culturally relevant care plan in collaboration with residents, families, and healthcare teams.
- Recommend care plan revisions and ensure referrals address all aspects of a resident's care.
- Maintain current knowledge and competency in nursing practice while considering safety, cost, and impact in care delivery.
- Mentor new staff and participate in their orientation.
- Demonstrate professional leadership and competency in the practice setting.
- Be aware of occupational exposure to blood-borne pathogens and handle potential contact with blood, body fluids, or tissues accordingly.
