POSITION SUMMARY:

The Case Manager is responsible for providing wrap around services to re-entry youth and their families and will be involved in all aspects of the program including intake, case management, and making appropriate referrals to services. 

 

DUTIES AND RESPONSIBILITIES:

  • Assess client readiness for case management services based on needs, eligibility and ability and willingness to participate in program
  • Create case plans for clients with short, medium, and long-term goals and the steps to achieve those goals
  • Provide direct services and referral services, including but not limited to, neighborhood-based case management, relocation and transition, educational support, housing, mental health, substance abuse treatment, tattoo removal, etc.
  • Complete necessary documentation, including: community engagement, daily activities, tracking services, and any other data collection required by the contract for service delivery 
  • Prepare monthly case-related reports including outcomes, successes and challenges
  • Advocate for clients with agencies and service providers to minimize barriers to clients receiving needed services
  • Work with parents/guardians/siblings and provide services to the family as needed
  • Coordinates all activities for the client and families with the various stakeholders in the person’s life including school officials, employers, probation officers, law enforcement officials, etc.
  • Actively participate in staff meetings, trainings, client reviews, internally and with partner agencies
  • Maintain confidentiality and comprehensive confidential case files
  • Establish and maintain collaborative working relationships with community resources
  • Provides workshops in the Juvenile Probation Camps
  • Performs related duties as required. *

Qualifications and Experience


Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:

  • Bachelor’s degree in a related field
  • 1 to 3 years of experience in related field
  • 2 years of experience working with high risk or probation youth
  • Commitment to the mission and values of the non-profit sector, with a passion for social impact and community service
  • Must have reliable transportation, a valid CA Driver’s License, and proof of auto insurance 
  • Must be able to successfully pass Live Scan (background check)
  • Required TB Test clearance

  • Handle public contact and relations effectively
  • Develop and maintain effective working relationships
