POSITION SUMMARY:
The Case Management Mailing Clerk/Coordinator plays a vital role in supporting the case management team by ensuring the accurate and timely processing of correspondence and documentation. This position is responsible for preparing, sorting, and distributing mail related to member cases, maintaining organized records, and assisting in the management of case files. The clerk will work closely with case managers and other staff to facilitate effective communication and ensure that all necessary documentation is delivered to members and providers. Case Management Clerk/Coordinator will also assist Care Coordinators with Health Risk Assessments and members coordination. Position is onsite – Monday-Friday 8:00am-5:00pm

ESSENTIAL DUTIES AND RESPONSIBILTIES:
To perform this job, an individual must perform each essential function satisfactorily, with or without a reasonable accommodation; including, but not limited to:
Fulfillment Accountabilities:
- Print all department correspondence.
- Receive, sort, and distribute incoming mail and packages.
- Prepare outgoing mail for distribution, including packaging and labeling.
- Maintain organized records of all case files and correspondence.
- Ensure accurate filing of documents, both electronically and physically.
- Communicate with providers, members, case managers, and external agencies regarding mail-related matters.
- Implement and maintain quality assurance protocols for mail handling.
- Review outgoing materials for accuracy and completeness.
- Generate and maintain reports related to mail distribution and case status.
Data Management:
- Ensure data integrity and confidentiality when handling sensitive information.
- Ensure compliance with organizational policies and regulations regarding confidentiality and data protection.
- Store all outgoing and incoming mail for CMS audits.
Coordination of Services:
- Assist case managers in coordinating services for members, ensuring timely communication of necessary documents.
- Assist case managers in coordinating services with providers.
- Provide support to team members as needed.
- Complete Health Risk Assessments.
- Advocate for patients’ needs and preferences within the healthcare system.
- Maintain accurate and comprehensive records of patient interactions, care plans, and services provided.
QUALIFICATIONS & EDUCATION:
Education: High school diploma or equivalent
Experience:
- Proficiency in Microsoft Office.
- Excellent written and verbal communication skills.
- Strong attention to detail and organizational skills.
- Ability to multitask and prioritize tasks effectively.
- Strong problem-solving skills
- Fluent in Spanish and English
- Member Service experience
