Position Summary:
This position performs direct outreach and engagement to unsheltered and sheltered homeless populations by informing, encouraging and assisting them in accessing appropriate programs, resources, services and shelter/housing. Intensively work with homeless individuals, most of whom have one or more disabling conditions, on a regular basis to facilitate linkages to a range of services and resources. This position also focuses on building relationships with individuals to help inform the formulation of mental health assessments and complete disability verifications that remove barriers and increase access to specialized housing and public benefits.
Essential Functions of Job:
- Provide direct outreach outside (year-round in all weather conditions) to engage the unsheltered and sheltered homeless population and perform crisis intervention, provide support, advocate, and connect the population to housing and other health and life skills related services and resources
- Design, implement and facilitate various types of support groups for people experiencing homelessness and those having transitioned to housing
- Regular street/outside outreach and travel to other shelters and streets in the region
- Perform behavioral health symptom management and de-escalation (e.g. mental health, addiction related, etc.) with individuals that are under-the-influence and/or experiencing mental health crisis
- Address the immediate survival needs of the homeless population including distributing food, water, clothing and connect them to other basic necessities such as shelter and emergency care
- Complete mental health and biopsychosocial assessment and disability verification for all clients
- Organize and maintain accurate and up-to-date paperwork, including client files and program reports
- Enter comprehensive individual notes and assessments into online database systems (HMIS-VESTA, Ehana, etc.)
Position Qualifications:
Position requires ability to work with homeless sub-populations, including individuals and families with substance abuse, mental illness, developmental disabilities, veterans, elderly and people with multiple diagnoses
Education/ Experience/ Other Requirements:
Master’s Degree
LCSW or LICSW is required and maintenance of licensure
Car and valid driver's license required
Supervisory Responsibility
This position has no supervisory responsibilities.
Work Environment
This job operates in an office and professional environment. This role routinely uses standard office equipment such as computers, phones, photocopiers. On occasion the job will require going out in events or organizations, based on the needs of the business.
Physical Demands
This is mostly sedentary in an office space but requires occasional local travel. This would require the ability to sit at a desk, frequently walk, bend or stand as necessary.
This position is primarily off base and will require field work under all-weather conditions
- Ability to walk up to three miles in a city setting, rural areas and in forestry.
- Ability to carry a backpack with supplies weighing up to 20 lbs.
- Ability to work outside in a wide-range of temperatures and weather conditions (e.g. rain, sleet, snow, heat, etc.)
Position Type and Expected Hours of Work
Designated days and schedule will be primarily Monday-Friday from 9am-5pm though hours will be based on the needs of the business and may require weekends, nights or early mornings on occasion.
Travel
Position requires minimal travel; based on the needs of the operations and business.
