Position Overview
The Patient Care Coordinator plays a vital role in ensuring the smooth operation of patient care processes. From managing records to coordinating schedules, this position supports the entire hospice team in delivering high-quality care to patients and their families.
Key Responsibilities
- Administrative Excellence
- Prepare and audit reports and documents for interdisciplinary group (IDG) meetings.
- Ensure patient care records comply with laws, regulations, and organizational policies.
- Maintain the availability of clinical forms and admission packets for team use.
- Patient Care Support
- Assist in scheduling field employees and rescheduling visits as needed.
- Review on-call sheets, maintain logs, and communicate critical information to team members.
- Enter patient care data into HomeCareHomeBase, ensuring accuracy and timeliness.
- Communication & Coordination
- Attend to phone calls from patients, families, and medical facilities, ensuring proper follow-up and visit scheduling.
- Coordinate care between disciplines, initiating and following through with care plans.
- Assist with educational activities as requested.
- Maintain compliance with federal, state, and local regulations, accreditation standards, and hospice policies.
- Perform other duties as assigned by leadership.
Qualifications
Education:
- Required: High school diploma or GED.
- Preferred: Licensed Vocational/Practical Nurse (LVN/LPN).
Experience:
- Prior hospice experience and familiarity with electronic medical records (e.g., HomeCareHomeBase) preferred.
- Strong knowledge of hospice requirements, including Medicare, Medicaid, and private insurance guidelines.
Skills:
- Excellent organization and multitasking abilities.
- Strong communication skills and a positive attitude when working with patients, families, and team members.
- Knowledge of medical terminology and HIPAA regulations.
Physical Requirements
This role is active and may involve standing, walking, bending, kneeling, and interaction with patients. Personal protective equipment may be required.

Pay: $25.00 - $35.00 per hour
Expected hours: 30 – 40 per week
