Essential Duties and Responsibilities:

The Care Coordinator will be responsible for conducting comprehensive patient needs assessments and coordinating with other healthcare professionals to develop personalized care plans.
Essential Duties and Responsibilities:
- Coordinate patient referrals, HEDIS measures, prescriptions, and care with precision, detail, and accuracy.
- Collaborate with each department to ensure patients maintain outstanding health conditions.
- Prioritize patients in the Primary Care Physicians panel, expedite orders, and complete all HEDIS measures for assigned panel members.
- Track assigned panel members and identified high-risk patients, monitoring their health status.
- Call high-risk patients 1-2 days before the weekend to prevent unnecessary hospital visits.
- Monitor daily ER visits, hospital admissions, and discharges.
- Schedule patients within 72 hours of discharge and ensure all hospital records are in patient charts before appointments.
- Communicate with providers and lead coordinators about hospital admissions/discharges.
- Document all activities in the patient's chart.
- Schedule specialist appointments and notify patients of the details.
- Track new HMO patients and schedule initial visits to capture all diagnoses, medications, and specialists' information.
- Pull member rosters on the 1st of every month and call new patients to set up appointments.
- Follow up with disenrolled patients to understand the reasons.
- Track patient information via spreadsheets.
- Be available one week per month for on-call services for HMO patients, using a provided laptop.
- Introduce yourself to the assigned panel members and provide them with your cell phone number.
- Use resources like Availity and Compass to generate necessary reports.
- Complete home health, DME, hospice, and prescription orders with proper documentation.
- Actively participate in daily physician/clinician meetings and communicate their needs.
- Enter all inpatient and outpatient elective procedures.
- Ensure missed appointments are rescheduled and communicated to the physician/clinician.
- Participate in Super Huddles and provide updates on high-priority patients.
- Address referral-based phone calls for the Primary Care Physicians panel.
- Complete phone messages and refills within 24 hours.
- Retrieve consultation notes and hospital records as needed.
- Follow up on home health and DME orders to ensure patients receive services.
- Track in-house DME supplies and email the lead coordinator weekly with supply logs.
- Provide extraordinary customer service to all internal and external customers, including patients and team members.
- Utilize patient messaging tools.
- Work collaboratively with other care coordinators and the MRA team to ensure patient satisfaction and good health outcomes.
- Additional duties as assigned.
Knowledge, Skills, and Abilities:
- Must be detail-oriented, possess the ability to multi-task/problem solve, and be open to cross-functional training.
- Exercise proper phone etiquette and navigate computer software systems proficiently.
- Be team-oriented and work well with patients, colleagues, physicians, and other personnel in a professional and courteous manner.
- Have exceptional organizational skills with the ability to prioritize and complete tasks in a timely manner.
- Knowledge of medical terminology, CPT, HCPCS, and ICD coding preferred.
Qualifications:
1-2 years of medical background.
MA certification or associate degree in medicine.
One year of internal medicine background.
