Soni Resources is looking for a dynamic individual to join our team as an Intake Specialist and play a vital role in shaping a client’s journey with our organization. You’ll be the face of the company, promoting our services while providing exceptional customer care. Build and maintain relationships with referral sources, patients, and staff to ensure seamless transitions into home care services. As a subject matter expert, you’ll guide clients through the admissions process and serve as a key resource for home health care services. Your success will be measured by client satisfaction, referral conversions, and monthly admissions goals.
Responsibilities:
- Provide clear, helpful information to clients and families about our home health care services to encourage enrollment.
- Develop new referral sources by gathering necessary client information and guiding them through the application process.
- Meet or exceed productivity and performance goals.
- Register caregivers, explain training requirements, and schedule necessary training.
- Deliver outstanding customer service and quickly adapt to changing needs.
- Collaborate with nursing and scheduling to ensure timely patient services.
- Assist with marketing, offer suggestions, and participate in community events.
- Maintain a CRM database of contacts and referral sources.
- Support other departments as needed and maintain confidentiality of patient information.
Requirements:
- Strong verbal and written communication skills with the ability to listen, clarify, and persuade.
- Highly organized with the ability to prioritize tasks and manage time effectively.
- Compassionate and empathetic customer service approach, especially in healthcare matters.
- Proficient in Microsoft Office, EMR systems, and other software tools.
- Able to work independently while thriving as part of a team.
Education & Experience:
- High school diploma or equivalent.
- Exceptional customer service and communication skills.
- Basic computer proficiency and data entry experience.
- Preferred: 1 year of experience in home care or healthcare, marketing experience, and familiarity with home health care software.
Compensation: $40,000-$50,000 a year
Salary is based on a range of factors that include relevant experience, knowledge, skills, other job-related qualifications.
